Sloan OEM repair part. This is the black rubber handle seal for manual Sloan flushometers including Regal, Royal, Naval, and Crown. Goes at the inside end of the handle assembly.

If there is water leaking from the handle, especially at the beginning of the flush, this seal may need to be replaced.
